Globalisation, ageing and the role of health services

The global demographic and epidemiological scenario is dominated by a rapid growth in the incidence of chronic diseases, both in high-income countries and those with medium / low income. The higher incidence of chronic pathologies in a context of increased average life expectancy produces a higher incidence of complex patients, who present the association of several pathologies and/or the co-presence of unfavorable social conditions (health and social vulnerability). This type of patient needs integrated interventions at different levels. The new epidemiological scenario represents a threat to Health Systems. This epidemiological context foresees an active intervention of the primary care sector in the coordination of interventions in the intermediate care sector, with a multi-service integration function between hospital and territory, and multi-sector integration between social, health, community and family resources.
