Descending necrotizing mediastinitis (DNM) is an acute infectious process arising from complications of cervical or dental infections. Former literature reports indicate up to 50% of mortality rate — this index has decreased since antibiotic therapy era. Nevertheless, the condition may still evolve to sepsis and death due to its potential severity. The aim of this study was to report a clinical case and to review scientific literature on DNM that occurred after teeth extractions. Twentyone- year-old patient presented odontogenic infection after extraction of the elements 38 and 48 evolving to DNM, which resulted in death even after invasive surgery and antibiotic therapy. In conclusion, immediate proper treatment of DNM is imperative, since its complications might lead to death.
