Early cardiac rehabilitation in acute myocardial infarction patients

Abstract

Studies have demonstrated that cardiac rehabilitation (CR) is effective in acute miocardial infarction (AMI) after hospital discharge. However, it remains unclear if early CR is effective in AMI. The aim was investigate if the early CR in AMI is effective, identify how long supervised CR can be started after the AMI event, identify exercise intensity and modality as well as indications and side effects for exercise prescription during hospital stay. The greatest benefits occur when CR is indicated within a week. Studies that started CR in the first 24hrs showed improvement in autonomic heart rate modulation, without hemodynamic effects or exercise intolerance. Improvements in quality of life and functional capacity have been shown with CR after hospital discharge. In-hospital CR is beneficial, it should be initiated as early as possible, and is related to better recovery. However, further studies should be directed to the Phase I CR to better understand the exercise prescription and possible reductions in re-hospitalizations and deaths.
