Ischemic stroke as a delayed presentation of traumatic bilateral carotid dissection: a case report

ABSTRACT The traumatic dissection of the internal carotid artery (TDICA) is a highly underdiagnosed disease. Seventeen percent may remain asymptomatic for periods longer than one day, and the occurrence of clinical symptoms after two weeks or more is not frequent. We report the case of a 35-year-old patient who had been hit by a car two weeks before admission. The same patient was hemodynamically stable, Glasgow 10, with right hemiparesis and motor aphasia. The CT scan showed image of hypodensity in temporal, parietal and left thalamus. Arteriography of the carotid-vertebral system showed bilateral dissection of internal carotid arteries. This report tries to get the attention to a disease that can lead to severe sequelae. Further studies are needed to address the creation of clinical protocols to diagnose and follow patients with strong suspicion of TDICA.Downloads
